Job Description

PRIMARY DUTIES AND RESPONSIBILITIES: 

  • Recording pertinent information pertaining to the client’s care.

  • Reminding clients to take medication at the appropriate time.

  • Planning and preparing meals according to specific dietary requirements of the client, and if necessary, feeding the client.

  • Assisting the client with therapy exercises under the direction of a Registered Therapist.

  • Assisting or performing personal care including hair care, oral hygiene, bathing, grooming, and dressing.

  • Assisting with ambulation with or without mechanical aids.

  • Assisting with routine bodily functions: Example, toileting.

  • Maintaining a safe, clean and healthy environment throughlight housekeeping including changing bed linens, dusting and vacuuming, cleaning kitchen and bathroom, and laundry when caring for a client for CLAS (Home care).

  • Providing companionship and stimulation for the client including reading, walks, etc.;and accompanying the clientto doctors or other appointments when caring for a client under CLAS (Home care).

  • Grocery shopping or other errands when needed when caring for a client under CLAS (Home care)

  • Performing other housekeeping tasks as indicated in the care plan when caring for a client under CLAS (Home care)

  • Taking client’s vital signs and recording input and output as part of the care plan established by the clinician.

  • Observing and reporting changes in client's condition to the Care Manager/clinician.

  • Complies with documentation and reporting requirements as assigned.

  • Consistently demonstrates timely communication with agency staff, supervisors, clients, client family and representatives, etc. Promptly returns phone calls.

  • Participating in in-service education programs so as to meet regulatory guidelines.

  • Carries out duties and responsibilities of position in a manner that exemplifies excellent customer service.

  • Consistently represents all Celtic enteties to all internal and external customers with integrity and professionalism.

  • Encourages and maintains safety standards in the workplace and home.

  • Thorough knowledge of and strict adherence to Celtic Healthcare’s Mission, Vision, and Core Values.

  • Performs other duties as assigned by management.


Employee May Not:

  • Administer or pour medication as mandated by law.

  • Cut the nails of any patient

  • Assist client with any financial matters including but not limited to: writing checks, balancing checkbooks, withdrawing funds from ATM, using credit cards to make purchases, etc.

  • Perform any type of heavy housework such as cleaning stoves with oven cleaners, moving heavy furniture or appliances, etc.

Required Education - Technical/other training or better.


Required Experience
1. A CNA must have related work experience, which is verifiable through references
2. High School diploma or equivalent
3. CNA certification required
4. Must be at least 18 years of age
5. Be physically able to perform the duties of the position
6. Exhibit mature behavior & understand patient confidentiality
7. Be able to follow direction & report to the Care Manager/clinican any changes in client's condition
8. Have available reliable transportation to/from assignments
