Senior Services of Central Illinois Announces Free Dementia Care Support Through Medicare GUIDE Program

Senior Services of Central Illinois (SSCI) is pleased to offer support through the Medicare GUIDE (Guiding an Improved Dementia Experience) Program, a new initiative designed to help individuals living with Alzheimer's disease and related dementias remain safely and independently in their homes while providing valuable support to family caregivers.
 
The GUIDE Program connects eligible Medicare beneficiaries with a dedicated care navigator who helps coordinate medical care, develop personalized care plans, and connect participants to community-based services such as home-delivered meals, transportation assistance, and other supportive resources.
 
Family caregivers also benefit from the program through personalized dementia care education, caregiver training, and access to respite services, allowing them time to rest and attend to their own needs. Participants and caregivers have access to support 24 hours a day for questions, care coordination, and assistance navigating dementia-related challenges.


Who Is Eligible?
Individuals may qualify for the GUIDE Program if they:
     Have Medicare Parts A and B as their primary insurance coverage
     Have a diagnosis of Alzheimer's disease or another type of dementia

Participation in GUIDE is completely voluntary and provided at no cost to eligible participants. The program does not change existing Medicare benefits, supplemental insurance, or provider choices.


How to Get Started
Interested individuals or family caregivers can contact Senior Services of Central Illinois to learn more about the GUIDE Program, determine eligibility, and begin the enrollment process. A comprehensive assessment will be conducted, and eligible participants can begin receiving services shortly after enrollment.

Family members may initiate the process on behalf of a loved one and work with the care team to complete enrollment with appropriate authorization.
